What to Expect from our Booking Process
- Once contacted, our clinic will connect with you to discuss your individual situation and to answer any questions you may have
- Assessment duration will vary depending on your individual needs. We will do our best to help you determine the best path for assessment based on your recovery goals
- You will be requested to complete and return an Intake Questionnaire prior to your first appointment whether it is a screening or an exam. You will receive the link to our online Intake by email upon booking your initial visit
- We require the Intake to be completed and on file prior to your appointment as it is used to plan for the upcoming screening/exam. It guides the direction of assessment, taking into account your individual needs
- Please bring all of your prescription and non-prescription glasses, including sunglasses, to your appointment. If you regularly wear contact lenses, please wear them
- Assessments may require more than one appointment, specifically to determine consistency of response to lenses
- Dr. Loopeker will review his findings and treatment recommendations upon completion of his assessment. Many patients find value having a family member or friend present for this explanation
- A written report is available upon request (for an additional fee)
- You are welcome to bring someone for support
